Summons issue to defendant under which cluase

EXPERTS PLEASE GUIDE ME WHICH FORM I HAVE TO SUBMIT IN COURT TO REISSUE NOTICE TO DEFENDENTS.

The court issues summons to the parties under section 27, Order 5- CPC ; 

You don't need to submit any form if except Court asks for the perticulars of party to served.

If the summons are served by the party does not appear then court proceeds  in ex parte .

If the summons are issued but not  served then the advocate or party can request court for issuance of fresh summons;

I personally attend case up to Cross Examin & if necessary I will appoint Lawyer.

As  you mention that if Summons are issued but not served then party can request court for issuance of fresh summons, but for this request which Form I have to submit in Court.

I request you to please advice.
