Although it may vary fromstate to state.
Suvidha Centre under Dc of the area in some states prepare ration card. The procedure and process is well described and is simple.
Police in many states has also established Suvidha Centres.
A self declaration which a typist can prepare may be sufficient on which local police station can mark the registration of information of "lost ration card' in their diary/register.

What is the procedure for the new ration card?.
Since my Father is retired person, can I apply the new ration card on my name?
Should we need to intimate the police station?
