letter for new branch adress

hai,

     i have a computer business at surat, recently i have expand my business to vapi and valsad, for this i started new godowns there.....now what is the procedure to add this new adress in vat department's records. i was informed that i have to write a letter to vat dept. now i want to know to whom this letter will be written and what will be written in that letter ?

 

please answer as early as possible
