Notice reply to labour inspector for minimum wage violation

Hi,

Need assistance in format for a reply to labour inspector. The notice says that the employer of a maid has violated the minimum wage for their daily task and that the corresponding documents needs to be submitted to the Deputy Labour inspector on the said date.

The maid has no say on this however, need to know what set of documents needs to be produced and what should be the format of the document.
