Inclusion of spouse in ration card

I have to include my wife name in the ration card.My father in law has removed my spose name from the ration card but he has lost the deletion slip/letter.

Now how can I include my wife name in the ration card?

Do I need to submit any proof for including my daughter's name. She is 2 years old?

We both are born and brought up in tamilnadu.Now also we are settled in chennai only.
