Meet higher officials.

Issuing office can rectify the mistake.
kavksatyanarayana (Expert) 12 July 2017
The OBC certificate will be issued by Revenue Authorities. so you will request the authorities to rectify the certificate by enclosing necessary proofs in which place you obtained the certificate. the correction will be processed by the Revenue Divisional Officer/Divisional Revenue Officer in most of the cases. you have to submit application through the Tahsildar only.

Any error, that too, requires to be corrected. Take up the matter with the concerned authorities. If not successful, direction from a Judicial forum could be obtained.
T. Kalaiselvan, Advocate (Expert) 17 July 2017
If the error occurred due to the lethargy of the staff concerned or it is due to oversight then the authorities concerned will rectify the same, but if it is due to your fault then you have no option than to approach court of law with a suit for declaration of your correct name and a direction to the concerned authorities for carrying out the rectifications .
The jurisdiction of court shall be the place where the cause of action arose.
