Writ petition & affidavit format

Is there a prescribed format for filing a 'writ petition', a 'public interest litigation (PIL)', in the Supreme Court of India?

If there is, where do I find it? Is it appended as a 'form' appearing towards the end of any 'supreme court' act or rules?

If that is so, then I will probably find a format for the accompanying 'affidavit' also, in there.

Do let me know.
