What is forwarding from district judge in the attachment column of notary application?

I have been filling the online notary application form. After filling up the memorial, the attachment page is displayed. There are various attachments like matriculation certificate, LLB degree certificat e. One of the attachment says forwarding from district judge. However nowhere could I find the meaning of the same and what exactly is supposed to be attached. What is the meaning of this attachment?
