There is no prescribed format and the complaint can be filed supported by an affidavit, as in civil cases.  The bare minimum details are as follows

1.Descripttion of complainant:with full address.

2.Descripttion of opposite party with full address.

3.What is the service available and availed by C from O.

4. What is the consideration:

5. What is the deficiency?(Cause of action )

6.Details and copy of notice of such deficiency of OP

7.Response if any.

8. Limitation: 2 years from such incident. and also notice.

9.Jurisdiction (OP /the place where the COA occurred)

10.PRAYER;

Now every DF is having a help desk that helps consumers and even provides a format available with them.

No formats or samples are provided on this platform.

Drafting a consumer complaint is the job of a professional for which you will have to consult and engage a local lawyer.
