Exemption from witness

Hi.experts..

I want one of my PW to get exempted from witness on medical grounds since she is a 62 years old lady and unable to commute anymore..

Kindly guide me how to file an application for the same or any valid medical certificate is required?

But here NBW has been issued against this witness..becoz she could not present due to poor health..

So.. in this case do we have to move some application or something..?

Kindly clarify..
