FORMALITIES FOR VARIOUS TYPE OF CITIZENSHIP APPLICATION
FORM IN 5(1)(A) : Application Form for Indian Origin (By Birth / original Indian)
Prescribed Application is required to be signed before the Collector (in Mumbai the Chief Metropolitan Magistrate or a Collector in district)
i) 3 Color photographs
ii)Copy of valid Passport
iii)Copy of valid Residential Permit
iv)Oath of allegiance on Stamp paper.
v)Vouching certificate (which is in Form) signed by Indian Original. Certificate should not be signed by a) relative of applicant, b) agent or c) solicitor.
vi)Documentary evidence of Indian origin
vii)Chalan if Ministry of Home Affairs says.
FORM IN 5(1)(C) : Application from a person married to an Indian. Prescribed Application is required to be signed before the Collector (in Mumbai the Chief Metropolitan Magistrate is a Collector)
i) 3 Color photographs
ii)Copy of valid Passport
iii)Copy of valid Residential Permit
iv)Oath of allegiance on Stamp paper.
v)Vouching certificate (which is in Form) signed by Indian Original. Certificate should not be signed by a) relative of applicant, b) agent or c)solicitor.
vi) Chalan of Rs. 500/-
vii) Indemnity Bond of husband / wife
viii) Domicile Certificate / Copy of Indian Passport of Husband / Wife
ix) Marriage certificate in English.
FORM IN 6(1) : Application from a Foreign national. Prescribed Application is required to be signed before the Collector (in Mumbai the Chief Metropolitan Magistrate is a Collector)
i) 3 Color photographs
ii)Copy of valid Passport
iii)Copy of valid Residential Permit
iv)Oath of allegiance on Stamp paper.
v)Vouching certificate (which is in Form) signed by Indian Original. Certificate should not be signed by a) relative of applicant, b) agent or c)solicitor.
vi)Chalan of Rs. 2000/-
vii) Indemnity Bond of husband / wife
viii)Two advertisements in separate Local news papers for inviting objections ( format is enclosed in application)
ix) Two language Certificate (that applicant know 2 Indian language )
x ) Three Affidavits of other persons stating that they have no objection in granting Indian Citizenship - format is enclosed in application)
FORM IN 5(1)(d) : Application for minor child whose father and mother are Indian. (this application accepted in duplicate by Home Department directly on 9 th floor, New Administrative Building, Opp. Mantralaya, Mumbai 400 032)
i) 3 Color photographs duly attested on reverse side.
ii)Copy of valid Passport
iii)Copy of valid Residential Permit
iv)Copy of proof that his father & mother are Indian Citizen.
v) Vouching certificate (which is in Form) signed by Indian Original. Certificate should not be signed by a) relative of applicant,b) agent or c)solicitor.
vi) Chalan of Rs. 100/-
vii) Personal particular in Form V of child
viii) 3 Specimen signatures of Father / mother
