Contact your Regional Provident Fund Office and submit the requisite application form duly signed by Notary Public.

If your company is not existance, for withdrawal of Provident Fund you can approach to following personnal for signature on the P.F. Forms & submitted to the P.F. office.

1. Manager of the Bank where the member is holding an account.

2. The Magistrate

3. A Gazetted officer

4. Chairman/secretary/member of the Municipal

5. Any other officer approved by the RPFC

I agree with above experts and will give better advise to get it transferred to the new PF account you may be hlding so that you can earn more pension.
