If you have shifted residence to some new house and husband is not aware of the new address, then summons may be served to old address. If that is returned court will presume that you have not attended the proceedings and issue exparte orders. Even if he gives wrong declaration about your address, it is possible.

Yes. It is possible with the connivance of some court officials and some postal department officials.
