Name discrepancy: sale deed & identity proof: can i purchase

I am trying purchase a plot and present owner name in registered sale deed is mentioned as Treesa Chacko and D/O. Chacko.  However the actual name given in voter id: Thressiamma D/O. Yohanan chacko and ration card shows: thressiamma chacko.    And SSLC certificate shows Thresia Chacko.
Also old passport shows :Treesa Antony.  Antony is hushand name.
Now the owner has obtained "one and same name" certificate from Thasildar.  
1. will there be any problem legally later on if we mention in Sale deed Treesa Chacko (a) Thressiamma?
2. in general : what other precaution to be taken care...
3. further is any self affidavit required?  can Notary in Hosur(Tamil nadu) sign this affidavit as the owner mentioned in sale deed presently living in Kerala.

4. Can I issue DD in the name of Thressiamma?

forgot to mention that sale deed present one got registred during 1989.

voter id: obtained in 2011 and ration card in 2008.

SSLC certificate: 1969.
