Paralyzed patient's thumb impression medical certificate

If Paralyzed patient can do thumb impression alone, which need to be confirmed by any government doctor is enough??? Or government doctor along with neurologists need this for medical certificate???

1. IF the person is not capable of affixing his signature /thumb impression, THEN the same can be done before a constitutional authority or a gazette officer, for it to be legally effective.
