For central govt info please refer my files under civil laws. For state govt info, some of states have prescribed format for application, which is available on official website of concerned state or you can visit website of  state information commission. Different states have different amount of fee and mode of payment. For central govt U can enclose postal order of Rs.10/- payable to accounts officer of the govt office payable at the place of submission of RTI appln. Where no format is prescribed, you can safely use format for central govt given in my files.
