simply just change the name apply in govt. press charni rd bombay.just rs 10 is fees get notify in gazett.no need of disclosing the any of birth certificate. documents rare for a person ,person is not for documents. no. of indians have two birthdates most of them is 01- 06----- etc.

Why dont you consider changing your names to the names of your parents!! This could be a simple solution to all this complication

you rather first contact person who makes passport at mumbai.

surrender your childs current passport alongwith reasons showing for the same.

this will be timely process, 

then apply for newer passport.

in consultation with the regional passport office.
