Addition of surname in official documents

One of my client has her name as SUSHMA in her matriculation certificate and now at the time of her admission in post graduation course she is interested to add her surname as SINGH with her name SUSHMA .
Dear experts can any one suggest what is correct & legal process/ procedure for adding surname by which there is no problem in her future.

Sushma has to publish the changed name in newspaper for that she has to give an affidavit.
Thereafter changed name may be published in Gazzate.

@Dr JC Vashista and Dineshwar Singh Kaushik: Mr Vashista has suggested that an affidavit be made before a notary and the same be published before a notary. I submit that this is not the proper procedure.
The various boards of secondary education and the CBSE do not change names in the matriculation certificates on every application. The procedure is that the student or his guardian must prove to the respective board by way of school records that the name had been wrongly spelled in the matriculation certificate. If the school records, attested by the Block Education Officer, indicate that the name on the certificate is not what is present on the school records, then the board will effect the change.
However, if the name on the certificate is the same as that on the school records, then the only option is to approach the civil court and plead that due to negligence, a part of the name had been omitted, and request the court to order that the same will be modified. The school, the BEO and the respective boards need to be made parties.

Mr. Chishty please find that name of a person can be changed/modified at any stage, even subsequent to CBSE & or any other authority for all purposes as stated in my previous reply.

An adult has right to change his name any time s/he chooses to do so.
The question arises of procedure;in case of change of name, One should first make published advertisements in two reputed newspapers (one local newspaper of the area in which s/he is residing and 2nd in newspaper of the area of permanent address).As far changing the name in various educational certificates & degrees etc will not be so easily possible,you can keep the newspapers advertisement & copy of old/new passport with both old & new name to be attached with thecertificates/degrees etc wherever & whenever required to identify with these documents with you to be the real person with changed/new name.In addition a gazette notification may be more benificial.
