well these are the sufficient documents to file a case against the doctor.

These documents are unsufficient to file a complaint against a doctor. Get the opinion of higher medical agency that the treatment given by that doctor is wrong and against the accepted standards of medical science. Until and unless you possess this document with you, no complaint against medical practitioner is maintainable as per latest vedict of Hon'ble Apex court.

I agree with Makkad sir's opinion. If you want to proceed legally then, along with these documents, you also need to have some evidance in the form of other expert doctor's opinion in order to show that the treatment given by that doctor was wrong and you suffered because of it.

For the purpose of filing, the above documents are sufficient. When the case reaches the stage of Complainant's evidence, by that time, if it is possible for you to get an expert opinion from a doctor you may file his evidence affidavit. Otherwise, you may seek the assistance of court to summon an expert or to refer your medical records to an expert for which the court may ask you to deposit some amount. Hence you can initiate the process of filing the case and as the case progresses we can take steps for expert evidence.

S Radha Pyari suggested the right path. Practicallly it is very difficult to get expert opinion privately, against the doctor due to their professional fraternity.
