Passport office notice for birth certificate

Dear Sir,
I had requested to issue Passport of my daughter two years back. We have submitted Birth Certificate as ID proof . Birth Certificate contains Parents name & registration number from Local Municipal authority.Birth certificate was issued w/o child name.
I have filled name in Birth certificate and Passport has been issued for my child. Recently i got Show cause Notice from Passport office that Name in Birth certificate is not matching with Municipal data.Hence Birth Certificate is fake and need to surrender Passport.Birth certificate is guanine with municipal registration number & Date . I didn't register the name of my child in Municipal committee because i was not aware on Process.Later we have updated same name in Municipal committee after getting notice from Passport office.
Passport is already surrender to passport office two months back .
Pls suggest , how to go ahead to solve issue

make application for fresh passport but with updated details .
please disclose that you had earlier been issued passport but it was surrendered after issue of SCN
