Passport application issue (name change)

Hi experts,
I got married in 2012 n when I applied for my passport including my wife's new name instead of pre-marriage name. Now I was about to apply for her fresh passport but now I was wondering that due to her all the address proof's n educational documents I was thinking that I should change her name on my passport with her old name rather than post marriage name. I don't know how to do that, please guide me what should I do.. I have the Marriage Certificate with pre-marriage name alias post-marriage name. Please let me know what all the documents I need to furnish and which form I need to fill it up.

Along with the application of passport, file your marraige card and an affidavit of your wife and some other related forms which will be available at the office of passport office. They will give free of cost.

Yes, along with the passport application, a marriage affidavit affixed with joint photograph as per the annexure (format of which can be seen in the passport application itself), duly notarized should be submitted which will suffice the requirement.
