Could any expert please provide me the format of a PIL against the malpractices of a religious institution? Is it advisable to file the writ in the HC first or directly to the SC? Please also provide details of procedure for both the cases.

no special format for PIL, the ordinary writ format will do.

but prior to moving PIL make sure your facts are true, clear and fit for a PIL

if the entire idea is to gain popularity out of the issue then be ready for the high costs to be imposed by the court.
