Name difference in school certificate

Dear Experts,
My official name in all records is PKL but father's name mentioned in my Son's X Board certificate is PK. Surname is not there.
Now my question is who should give an Affidavit?
Should I declare that PKL and PK are one and same persons vide an affidavit? or
Should my son say in the affidavit that my father's surname is missing in the certificate and should be taken as PKL.
In either case is it necessary to get the announcement published in the newspapers?
The above is required for obtaining passport of my son. I do not have a passport.

What is the name mentioned in birth certificate and other id proofs?

Try to get name changed in educational certificate if possible.

For passport he may proceed with the name as mentioned in educational certificate or by using both names.

Dear Sir,
The problem is not with his name in school records. The problem is that in the place where father's name has to be written the Surname is not there, whereas in Birth certificate the surname exists.

Kumar sir, execute an affidavit that PKL and PK are one and the same person and also it it better your son may also execute an affidavit that my father surname missed in the certificate but PKL and PK are one and the same i.e.my father. if you are employee, obtain a certificate from your employer also.

Does it affect any where in the records, proceedings, documents? I think it should not affect and let it remain untouched.

However, if it affects, write to school authorities with a sworn affidavit.

Can any one send a sample affidavit and also if some expert can please let me know the value of Non judicial stamp on which such affidavit is to be executed in Karnataka.

I agree with Mr.Vasishta. Such things are common and do not pose any problem in the normal course. And, if at all any, your son can solve them through an affidavit.

any Notary can make the affidavit if you say the details for what the notary is required. however you can get online.(www.vakilno1.com)

Dear Experts,
Yesterday my son had been to passport office and submitted along with the required documents, an affidavit stating that PKL and PK are one and the same person. The affidavit was also notarised.
But the Verification officer says that the affidavit has no value and hence could not be accepted.
Can anybody suggest a remedy?

Have you asked the way from the verification officer or tried to meet the passport officer?

Dear Rajendraji,
The verification officer wants the BIRTH CERTIFICATE to be changed. He is of the opinion that this is the only way out.
Does affidavit sworn before notary has no value - Your legal opinion on this, Please.
Any other feasible solution?
