Format of application for commission to record evidence

If any of the learned lawyer or professional could provide the format in which I should put an application for commission to record the evidence in district consumer court, i will be very thankful. Kindly provide a sample format, if available.

Where the value of the goods or services and compensation exceeds Rupees from 20 lakhs up to one crore, the complaints can be filed with the Registry of State Commission. A complaint should be accompanied by the necessary documents and verified affidavit of the Complainant. 1. Required copies: 4 + Sufficient additional Copies for each Opposite party.
2. Limitation: Complaint filed within two years from the cause of action arose
(personal/legal notice must serve to another side before filing the Case)

There is no specific format to record evidence.
Evidence in each case differs, it is advisable to consult a local prudent lawyer with relevant records.

Thanks Dr.! Will do accordingly. Actually i was complainant and party in person, in a consumer case. Hence was seeking the format, as the court directed "to file application for commission to record evidence" after submission of proof affidavit by me.
