Name change

Hello Sir,

I had changed my surname after my marriage through an afidavit but have lost it.

I want to know if I can change it again back to my original name.

If I cannot change my name,then what should I do to re issue the lost certificate?

Definitely you can change the surname to your original by an affidavit sworn before a Magistrate and thereafter make a public notice by an advertisement in a news paper.

tell us (as Vishal said) whether the proper procedure was actually followed prior to name change or not.

is there any specific reason for change in name again.

any official documents reconstructed with changed name?

hi,
the procedure for changing the name is to publish it in the official gazzete of the state(from is available in the office) then to give the public notice in the local language newspaper and in english newspaper. then u have to make an affidavit to that effect. but in ur case u had not follow this procedure so evenif ur affidavit is misplaced or lost that doest make much difference coz u had not follow the legal procedure.

As I mentioned that I lost the afidavit,hence I want to change my name back to the original one.

While changing my surname,I did fill up a form.I also published it in an english daily.

I have not reconstructed any documents with the changed name.

Can I apply for the change of name back to my original one anywhere?I changed my name in a different city and currently staying in a different city.
