Hi, my wife is doing a gazette notification for her name change after marriage as she adopted my surname. Her old name was Deepti (no last name) and her new name post-marriage is Deepti Rath. The affidavit that we made starts like:
"I, Deepti Rath, daughter of xxxxx xxx and wife of Shakti Ranjan Rath, aged xx years, residing at xxxxxxxxxxx, do hereby solemnly affirm and declare as under:
That my name before marriage is Deepti."
Please suggest which name she should sign on the affidavit: old name (Deepti) or new name (Deepti Rath). We are planning to produce her driving licence as ID proof which has her old name as it was prepared before marriage. However, I have already made her PAN Card and bank records with new name using our marriage certificate.
