Sale Certificate format under SARAFAESI Act, 2002

Please if anybody can provide me Sale Certificate format under SARAFAESI Act, 2002. As per Apendix V of the Rules

It is already contained in the appendix of Act. If you have booklet of the act then you may find it easily.
