FSI & PF scheme applied on such employer under More than 20 worker working.
As per your note your salary is Rs.15000/- Per month but you have to provided. It seems that 12% of your salary debit as PF & others as ESI.
You may contact to your employer's accountant he will disclose.
You may also see it at your salary slip- if employer provided.
You may also check from ESI office. OR find the details using RTI Act.

As per latest amendment to ESI Act, establishments have to be covered if the total employees are 10 and above. Out of gross salary, employee contribution shall be deducted from the salary at 1.75%.
