Hi everyone,
My father has changed his name in his legal documents (i.e. - Driving License, Ration Card, electric bill).
Now as his old name is on my passport i need to change the same on my passport.
he also has affidavit done from gazette so what else would be requried to submit to passport office so that they can do the name change in my passport too.
