spelling mistakes

in my metric cert my name is hemanth nandlalji gwalbansi and in my higher secondary my name is hemanth nandlal gwalbansi but my is sarika nandlal gwalbanshi.what i have do to prove my name is hemanth nandlal gwalbanshi.

Relate with berth certificate, school admission forms and onwards.. If there was no error/name change then for correction check with O/o School/Board..
