Talaqnama dissolution of marriage & adoption of child-help!

Hello Lawyers!

Here is brief about my case and queries below:

I married in 2006. I am Hindu married to Muslim man.

We have Nikahnama and registration of nikahnama.
We did not register under special marriage act. 

I seperated in 2013 July and divorced in December 2013.

He is already re-married in Feb 2014. And settled in chennai.

We have Talaqnama. (Mentions nothing about the child).
And we also have Memorandum of understanding (notarized, not registered). [ MOU clearly mentions he has no rights toward the child]. But I am not sure if the law will consider this document for further needs.

We have no other documents from court about dissolution of marriage.


Queries:
I have a daughter currently her age is 4.9 Yrs.

(1) I want to change my last name (my maiden name).

(2) I want to change my daughter's first name and last name as my maiden name

(3) I am planning to remarry and the boy is ready to adopt the child and knows everything about the past. What all documents do we need for the adoption.

(4) I also want your suggestion, should we change the child's name now? Because again after adoption her name will need a change to father's last name.
