Passport annexure b on plain paper or official letter head?

Hello all,

My wife is doing job as Teacher in Pune,Maharashtra. She is a Govt. servant and have given application for acquiring a fresh new passport.
My question is that who can sign Anexture B certificate for Passport and on which paper we should type the contents of Annexure B and Annexure I (on office Letter head or Plain paper)
Please advise.

Annexure 'I' is the Standard affidavit to be Executed on Non-Judicial Stamp Paper and attested by Judicial Magistrate/Executive Magistrate/Notary Public

Annexure 'B' is the Identity Certificate needs to be print on the Letterhead of the Concerend Authority and the Concerned Authority only has to put a Seal and Signature on it.
