after marriage i continue to use my maiden surname. however on my babys birth certificate my husbands surname was put for for name and the sam egot endorsed in my babys passport. now what document should i have to say that both are same persons. i require that for visa purposes.

you and ur husband execute affidavits regarding your baby name and the affidavits shall be notarized by a notary of your area. please consult the advocate who is a notary.
