Mr.Yogesh,

Every document should be attested. Attestation of an instrument should be made by two or more witnesses who have seen the executant sign or affix his mark  to the instrument. Each of them must sign in the presence of the executant. The purpose of attestation is to ensure that there is no fraud or other mitigating circumstances in the execution of the Deed.

in each state there are general rules (civil),and general general rules (criminal)for district courts and high court rules for high court.procedure for filing of documents are governed by these rules read with relevant procedural laws. in Allahabad high court, annexures are filed alongwith affidavit sworn by petitioner or his pairokar before oath commissioner.the petitioner or his pairokar has to put his signature on annexures in presence of oath commissioner. pls note, in judicial affidavits oath is admistered before oath commissioner appointed by the court and not before notary.
