Birth certificate after 30 years

Made a affidavit regarding issue the Birt certificate.

Fill the form.

Submit it before Birth & death Department/ Panchayat Office.

They will sent your application to SDM/City Magistrate. After inquary he will passed the order to issue Birth Certificate.

But Remember___________

If you born invillage area- than furnished application to SDM.

If you born in Urban area than application to City Magistrate.

I was born in a military hospital, so should i make an application to SDM or to city magistrate.

Also as of now i am in AP because of my job and my home town is ambala (haryana), where should i make this application, in my home town or i can make a application from AP where i am right now.

You can't make application in AP,

You have to furnishe your application with reason & notary affidavit to Birth & death department in Ambala city.

They will provide you your application with remark-and transfer to City Magistrate.

City Magistrate will call the report- After satisfy with your documents & report. He will order to issue DOB certificate.

I will be going to ambala in the month of ambala, 1 more thing i was born at Binaguri (WB). currently my home town and my  permanenet home address of ambala, same is on my Passport.

what do i do in this case.
