Hello,
My mother's name is Mrs. Sujata P. Murkar, Her maiden name was 'Shoshanna'. Her first passport was made before her marriage and so there were no problems. After her marriage, her name was changed and her passport had the name as Mrs. Sujata P. Murkar - her maiden name was not added. Now she wishes to add her maiden name to her passport as an alias. We would like to know whether there is any procedure to do so and how to accomplish it.

In other circumstances for change of name, the applicant (both male and female) should furnish:
(i) Deed poll/sworn affidavit.
(ii) Paper cuttings of two leading daily newspapers in original (one daily newspaper should be of the area of applicant’s permanent and present address or nearby area).

Originally the maiden name should have been mentioned in the application for passport. The relative instruction column is as under;
Column 2.2: Are you known by any other names (aliases)
If you are known by any other name (alias), put a cross in the Yes box and provide the details in Column 1of the Supplementary.
For example,if your name is Rajesh Bansal and your alias name is Raja, put a cross in the Yes box.If you are not known by any other name (alias), put a cross in the No box.

If the name matches with the educational certificates or the birth certificate only then the word alias should be inserted.

Thank you for all your replies.
When my mother applied for this latest passport, she had given the necessary documents - the deed poll and paper cuttings - even the marriage certificate which includes her maiden name. Although the said procedure was correct, the passport officers refused to allow the addition of her maiden name in her passport. Now her latest passport does not contain her maiden name.
The officers say that the provision to allow aliases has now been canceled and no-one is allowed to add aliases in their passports now. Both mine and my father's passports include my mother's two names. But when we tried do the same for my brother's (he's minor) passport, they refused as my mother's passport does not include her maiden name. Could you suggest any solutions?
P.S. We will apply to re-issue my brother's passport soon. What documents would we need to provide to resolve the above issue?
P.P.S. Again , thank you all for you replies. It helps a lot to guide us in the right direction. Thank you.

If the provision of alias has been deleted then there is no alternate to do as you desire except to file a writ before high court.
