It differs from State to State. In Tamil Nadu, the person has to make an application to the Government Press on a prescribed form and pay the prescribed fee [it is approximately Rs. 450], mentioning the present name and the proposed name and the reason for change. Some documents to show the present name and address are required to be attached. Once this is done, the notification for change of name is published in the Official Gazette, which is then sent by the Government Press to the applicant by post. In addition, a public notice by a news paper publication may be made [though not compulsory]. That completes the requirement. Whenever there is an enquiry about discrepancy in the name, the notification needs to be furnished. The process takes a month or two. The application form and instructions can be downloaded from the website of the Government Press [Tamil Nadu].

If you are in Maharashtra you have to apply to the Department of Government press to publish your change of name in Government gazette.

For ordinary publication it is Rs.120.
For urgent publication it is Rs.620.

In case of A.P.

you have to apply to the Executive Magistrate for change of your name.
