In Tamil Nadu the procedure is to issue a notice in newspapers that so and so will hereinafter be called as so and so. Then one has to fill up the prescribed form and pay the requiste fee and infomation about change in name will be public in Gazettee. The copies of Gazette can be used for authentication.

It is almost same in Maharashtra. The only difference is that the application to be sent to Government Press should be accompanied by affidavit.

In addition to the above procedure, if he is a government servant, you have to submit an application to your department authorities to alter your name in the official records.
However, if the change of the name (surname) in case of a women employee arises because of marriage, a simple application with the proof of marriage is sufficient.
Same procedure applies for deletion of surname in case of divorce.
