I want to know whether child care leave availed for 2 months can be extended further for 4 more months with medical certificate.

even without medical certificate it can be extended if sanctioned.
You have not stated as to whose medical certificate it is. It can be presumed it is medical certificate of employee herself.
However if the employee is herself sick then it is only a misuse of CCL. She has to take medical leave. Action can be taken even against sanctioning authority. CCL in such case should be denied unless the sanctioning officer is willing to sacrifice his /her career.
