Birth certificate

A lady's birth certificate shows her name, i.e. before marriage,to be different from the what she changed without any procedure after marriage. Her various documents like voter card, ration card and other documents show her name after marriage.
The issue of her got birth in his maternal parents home and the birth certificate shows name of his mother to be that what the name of her mother was before marriage. On the basis of the birth certificate the name of mother in the school records has been entered as what was before marriage.
My query is that what way should the mother adopt to get the name corrected in the school records to get her name after marriage incorporated in school records of her child as her main documents also show her name after marriage.

if name of mother had changed after marriage she must be having marriage registration certificate which would be reflecting her name after marriage .if her name has changed she must have got the name change reflected in official gazettee .

based on said marriage certificate/ gazette notification request the school to change daughter name in school records .

On the basis of educational certificates of the mother (Having maiden name)and marriage certificate the name of mother may be changed by the Municipal Authorities which issued the birth certificate.

Name was not changed by following any procedure and marriage was also not registered.
What remedy can can be availed in such circumstances.
