Process to get permission to meet child

My case for judicial separation is in process in the district court. my wife doesnot allow me to meet my daughter from last 6 months due to conflicts between us. what i have to do so that i can get permission to meet my daughter

Please file an application for visitation rights. Ask your lawyer, he/she will guide you.

Move to court to order for visitation rights.

please tell me that an application on a simple paper for visitation rights is enough or it have to be on judicial/stamped papers.
