If an employee availed Commuted Leave and Submit his AMA Consultation Bill for Fit and Unfit ... is it passed by competent authority or not ?

What is the real issue? You have not stated the complete facts. Producing fitness certificate on joining duty on medical grounds is one thing, and getting reimbursement of medical expenses is another. In order to get medical reimbursement you need to submit the claim in the prescribed form to the controlling officer through proper channel.
