Change of name in all documents

my father's name in my school and graduation mark sheet is syed ashfaq hussain whereas in his documents his name is syed arif ,cuz he's known as syed arif ,now we want to apply for passport and pan card but cuz of this name problem we are confused ,wat to do ?

what is name in birth certificate ? if name in birth , school leaving certificate are syed ashfaq hussain he can apply for change in name on other documents based on school leaving certificate . what is name in PAN card ? bank accounts , ration card ?

Can use the option of alias in name if the documents are with both names.

Other wise contact passport office.

on my school leaving certificate his name is syed ashfaq hussain , and on his PAN card,bank account its syed aarif
whats procedure of name change and wats difference between affidavit and gazette ?
and do passport office accepts affidavit ?plzzz help me :(
