Correction of surname in my certificates

Hi Sir,
I have been passed out from my school in 2007..starting from my school i don't have my surname on my certificates..which the actual name is "Sheri Sharath reddy" but unfortunately i didn't get my surname on my certificate which i have been given certificate as " Sharath Reddy" which is followed same as in Intermediate & even in Btech..But, in my Rationcard , Pancard I have the correct name along with Surname..Can u please suggest me the solution for this inorder to apply for my passport.

Write an affidavit suffixing your surname, get it published in 2 newspapers (English & local language)and get the same published in Gazzette.

Adding surname to the existing name is as good as change of name and the procedures meant for it can be followed.

I even contacted my school Mistress..she said me that she doesn't have the knowledge about that..I am really panicked about this issue...Vashista Sir I didn't get your advice sir..can u please elaborate it..
