Dear Sir/Madam,
We have received a show cause notice from director of industries health and safety in respect of fatal accident and death of a employee in our one of the factory. They write us- why not action taken against us and reply required within a week. Please guide in this regards urgently for reply this show cause notice.

Respected Sir,
First thanks for your reply to my query.
Our one of the unit located in maharashtra in jurisdiction of a village, before starting of rainy season some repairing need on roof. our team with senior officer goes up on roof without any safety measures for carried out maintenance work, after complete work when they move back to come down that time a fragil roof broken and our senior officer fall down and on the spot he was died. we do all formality like intimation to concern dept and other fomalities but now we received a show cause notice form industries of health and safety dept that state "why not legal action taken against you for violation of Rule 73(F) of Maharashtra Facatories Rules 1963". we form a committee for investigation and said report also submit to same dept.
Sir, above mentioned is full fact of accident in our plant. please inform what is process to settle this matter and how to write reply for this show cause notice.
