Correction in birth certificate of my son

How can I change my Wife's name and also correct the spelling of my Son's name in his Birth Certificate(now his age is 9yrs) which was wrongly mentioned?

Contact the local birth registration office, submit an application along with affidavit and other required proofs and get the needful done.
