you have to raise a specific query in respect of which legal opinion is sought . a fishing expedition wont serve the purpose

There is no format prescribed by law for giving a legal opinion. You will come across banks prescribing formats but all these are need based. The details they want their advocates to mention will be put in a format so that the bank can read the opinions properly for taking sound decisions.
The purpose of any format is only to ensure that all the required particulars are mentioned in the document.
The formats may vary depending on the points which are important for the given issue. Hence there cannnot be a universal format.
