hello sir, i need little advise for my wife passport re-issue.My wife surname changed from Hemlata Bisht to Hemlata Rawat after marriage as per Marriage certificate but her old passport and other documents (like 10th certificate) has only Hemlata in them but no surname mentioned.Only adhar card has Hemlata Rawat as her name.
Passport office rejected the request to re-issue the passport and asking for another address proof and annexure D.Their helpline is guiding us to get Annexure E.We are confused which annexure is required.
She don't have ration card and there are some mistakes in voterid card also.
Please help!!

get registration of your marriage and produce the same.

You can show the marriage certificate if her new surname is mentioned there.

Send them the required documents i.e another address proof and annexure D.

You may make an affidavit on the basis of the marriage certificate or any other relevant document mentioning her original surname, get it notarised and submit it along with the requisite annexure.
