Guardianship Certificate Format

Can any one of you help me if there is any format of guardianship certificate to be issued by the district judge to the Guardian of the Minor. Please its urgently required by the District Judge: If not, can you prepare the certificate for the Guardian to be issued by the District Judge?

Dear Assumi,
In UP I never heard about such a certificate or format. Here the DJ or PJ of Family Court passes his judgment on the petition of guardianship & the judgment so passed is used as a certificate of guardianship everywhere.

Sir as Sri Sanjay said A GOP seeking guardian
ship of only minors will be usually filed before Family court and if you seek guardianship of minors as well as the property then the same will be filed before DJ and after cause publishing the paper publication and examination of the Petitioner the Court will pronounce the judgment.
