Thanks for looking into the matter. How can I express what currently I am feeling. Only 1 day ....Its only after 1 day of my daughter's Death, Since then till now I am struggling with reliance to get answers of my questions. My daughter expired on 29th Dec 2010, since 30th of December I am continuously contacting Reliance General Insurance on their carelessness and wrong doings. These people are so cunning that they are continuously ignoring my emails and phone calls and not replying to a single question that I am asking. Instead of that they have sent me a check and only email that says "Claim has been settled".
I was in deep pain ....and to my surprise I was continuously writing complaints to IRDA, and IRDA ignored everything ........This raises my eyebrows over the authenticity of IRDA. I thought if this is a new SCAM in the country, first Telecom now Insurance sector.
Here are chain of Incidences for your quick understanding into the matter.
1. On 30th November 2010 I renewed my Old gold plan policy and was convinced by executive on phone to switch over to standard plan(Health wise and accidental). I did a credit card payment of Rs 12991, and just got three SMS as the confirmation token.
2. On 27th Dec 2010 my daughter got injured in an accident, she was in emergency and I was informed by my family members that I might have to take her to Mumbai if we want to save her life, but eventually I couldn't and we lost her. I thought of expenses at that moment...I knew that I had recently included her into Health wise policy ..so it would be okay on the expense front .....but next moment I got shocked when one of my cousin told me the TPA number(Policy number) that you have given , is not working ....I was speechless ...it was the moment I can not forget. I had already paid money for a Health wise policy and I was not able to use it when it is required badly. I realized at that moment that I have not received any policy document yet, and maybe they have not even renewed the policy . I was under great shock...all I had was just one credit card transaction done to RGI.
3. I had this doubt in my mind that, reliance might have not created the policy, and may be they have not included my daughter in the policy. On 30th December I try to check the call center ..and asked about the policy ..They didn't say anything about it ..and kept on saying you will get your policy soon.
4. On 3rd January ..for the first time we got a soft copy of the policy through email. It was the biggest jolt to my life .....I fell down on ground when I saw the policy document which had the contradicting statements.
I am attaching its snapshot here...
Seeking for the clarification we kept on writing emails to services.rgicl@relianceada.com & sudarshanam.sundararajan@relianceada.com & complaints@irda.gov.in till 12th of January. To IRDA we kept on sending the complaint in the format asked by them, but we never got any reply of the complaints registered.
5. I had made a transaction of Rs 12991 through Citibank on 30th November. In the premium certificate I can see reliance has smartly swollen the taxes and it has only given me the premium certificate of RS

6. I had filed n number of complaints to reliance customer care, and I shared all the reference number with them on each email ....but no reply on any of the complaints ...those things are simply ignored by reliance.
They are treating us like a street dogs ....no respect for the customer.
7. I kept on asking about the hard copy of the policy, I got the first copy of the policy on 21st January and 8 copies on 28th January, I have still not got any reply on the delay in the policy. No body is saying anything, they read all my emails, but never reply to any of my questions.
8. I never asked for any claims, it was on 18th the first time I declared that my daughter is dead, and I apply online for the claim(Just because they have a policy to claim within 30 days). I got the first ever email reply from Mr.. Harish from reliance saying sorry for the loss ... we will process your claim very soon. I never asked to process the claim ...I kept on asking the questions, and want clarification on that. No reply to any of the questions ......and they cunningly sent me an email saying your claim has been settled.
Reliance sent a representative @ my place. I thought may be he is having answer to all my questions, to my surprise he said we will send you an consolidated email reply. That consolidated email has not yet been received .....
We got a "FATWA email " from reliance which says "Claim has been settled".
On top of that when we asked for a legal action against their wrong doings ..we were threatened by Reliance that do what ever you can .....you will not be able to move anything. Its reliance .....
When we asked about the contradicting format ....we were told that it has been approved by IRDA. My question is ....Is it really true that IRDA has approved the above mention format of the policy ....
Or shall people still have faith in IRDA ....that it will take some action against the SCAM running by reliance ........
I have even dropped so many emails to IRDA ...begging for justice ...But to me Reliance and IRDA looks like a two sides of a coin.
I then decided to write a separate email to all the top officials of IRDA ...to bring this scam running my reliance ..under protection of IRDA( The name that Reliance is using as shield)......I am sure if common public will hear such kind of news ..they will be shocked .....
I was overwhelmed when I got a reply from you ....I still have some hopes .....I will wait for Justice ..........

1. We got a courier containing Three papers of the policy. Dispatched date 15th December ( As per IRDA policy you should have sent it with in 15th day of policy subscribed).
2. We were never ever informed about this courier being sent on above said date and bounced back. We upfrontly asked your customer care executive about any courier being sent and bounced, we always got a NILL reply. The latest we got was about the courier being sent on 5th Jan From Express IT, which has not yet received and no body is saying anything about it.
2. This courier is marked as bounced on 22nd( No body was at the address ). Not possible, we can show you the proof of other letters received on the same date. For your kind information we always have a 24X7 security person appointed there.
3. Even if i take your point 2 as true , Doesn't your courier company understands how to dial a phone number. Even a simple uneducated person will visit the delivery site again and at least will drop a call. And that is why Phone numbers of receivers are mentioned on the Envelopes.
4. Since 22nd December till now, none of your executive have any information about this delivery [ STRANGE ].
On top of it ....When i shared previous email about the continuous follow ups ...I haven't yet got any reply. Let me know how long will you take to investigate this case .....

Sir, I am again telling you this ...the way all the things has happened " I don't trust reliance" , i can not handover all the original Documents to reliance. I can only do this once this all get settle.
I have taken a legal advice on it ....and things can't not be taken for granted. If you can not send the Original Policy document within 50 Days of generation of policy ... how would you keep our original document safe, i doubt if it will reach to your office.
What max i can do is ...i can send you scanned copy of all the original document and also a attested copy of the document. Once matter get solved i will submit them in person.
Also, the person you are sending should carry a valid Identity proof and a letter ( With Reliance letter head) stating he is authorized to collect the document.
There are series of Ignorance and insult done to us .... i am very very very disappointed from the reliance behavior.
Hope this time i would get a quick reply.

Dear Sir,
I am devastated, broken intopieces not finding even words to express how bad I am feeling. Me and myfamily are passing through the most toughest time of our life. We lostour Little Angel "Prakrati Bhandari"(One and half yearold) in an accident she expired on 29 Dec 2010. The old sayingis coming true to me "Sukh meinSabb Saathi, Dukh mein Na koi". All the promises, attention & care given to the customerat the time of selling any insurance product suddenly disappears at thetime of providing the services. It seems like the sole motto of these bigorganizations is to gather the hard earned money of common man under name of "Insurance"(Scam). Nobody realizes / experiences theheight of negligence carelessness and in human behavior shown by RGI untilthey haven't met any miss happening within the family.
I am very unfortunate in thisregard, I got the real taste of life at very early stage. Got the trueface of reliance when this accident happened to my daughter. Here is thechain of incidences
[ 1 ]. I had anold policy with reliance PN. 282510330996 Dateof expiry 21/11/2010. This was the "Goldplan" , before the expiry of the Policy I got first callfor renewal in 22nd October 2010 and till 30thNovember 2010 I was continuously getting call for the Renewalof the policy ( See their promptness, politeness and caring nature ).During this period customer care kept on convincing me to go for standardplan. What I convinced from those conversation is that, Thepremium of gold plan is higher, I can get same facility if I go for standardplan for whole family. [ Note during this I was not even mention about theconditions, and any warranties ]
Through phone only I was convincedby customer care executive (Renewal department) to do a payment for StandardPlan which includes Health wise policy and Accidental policy for wholefamily. On 30th November 2010, I did a payment through my Citibankcredit card whose transaction detail is listed below.

As a token of receipt fromreliance, I only got three SMSes containing Policy number. Iwas not even aware that old card number for the health wise policy is notoperational, It was not even confirmed that my daughter is includedin the new policy or not. Since30 November 2010 till Now (16th January 2011) I have not yetreceived any Hard copy and not even New card under Health wise policy. Insunny day scenario this negligence is digestible, but in the rainy day(As it happened with me ) ? Should this be taken as excuse or matterto forgive ...certainly not this negligence is a criminal act.

[2] On 27th December2010 my daughter one and half year old "Prakrati" shehad a serious head injury in an accident. I was shocked withthe incident, couldn't see her in ICU when she was fighting between lifeand death. I was in such a big shock that I was telling everybody ....nothis has not happened with me....It is a dream...a Bad dream.....pinchme wake me up ..... this is not real. Seating in front of ICU ittook me some time to realize- Yes she is my daughter andit is a real incident ......I felt like somebody is drilling intomy heart .....a big pain inside .....I was crying like anything .
Just to provide her best possibletreatment we took her from Manu hospital to SantokbaDurlabji hospital in jaipur. I remember, that moment my brother asked me, "Do you have any medical insurance /Health policyfor her" I was speechless. Because I hadno answer for that. What I remembered at that timewas only one thing that I did a transaction of ~13000Rs to Reliance General Insurance on 30th November 2010. Iwas not even sure if She ( My daughter ) was finally includedin the policy or not. As I had no written proof / document which says "Prakrati"is also insured and it is confirmed. What was handy to me at thattime was only the OLD health wise policy card number 282510330996. Iprovided this card to my cousin to check if we can get cashless onit [ *Please refer section [ 1 ]for the detailstory of policy ]. I was told that policy underthis card has already expired on 21 November 2010. I felt like "Hiroshima- Nagasaki", somebody has nuked us. ICried in hospital as if I gone Mad.......Shouted with Anger"GODPLEASE SAVE ME FROM ALL THIS ......." I stillremember it .......
Sir, I am feeling very shameto share this .... I had very little bank balance at that time andeven today....I decided to put my Gold engagement ring as a mortgage toarrange the money for the treatment. All that I wanted is mylife, my little Angel...back again. We were discussing thepossibility of taking her to Mumbai for better treatment, Iwas ready to cross all the limits......Just wanted to save her. ButI surrendered on the expenses front, I compromised with the treatmentbecause I couldn't afford the expenses to take her to Mumbai and get treatmentin "Bombay Hospital". I lether go .....because I am a poor father ...who couldn't saveher daughter due to financial limitations. Couldn't provide her bettertreatment.......I thought I don't have right to live ......But do you thinkI am the responsible person for this Loss....Do you think I could havesaved her .... Yes.....Had it been some other Insurance Policy with some other organization.....My daughter would have got the better treatment. I got screwed upby Reliance ...and still getting it ......
Sir, I knew about myfinancial limitations right from beginning ..... and realizing theuncertainty of the life in current world I opted for a Health wise policy & accidentalpolicy. I did this for my whole family, Me my wife and my daughter. Inold policy only me and my wife were included. In November onlyI included my daughter under the insurance plan & accidental plan.Till the time of accident, I had nothing in hand to know if my whole familywas covered or not. This is simply a case of Negligence .......I mustsay "RELIANCEIS RESPONSIBLE FOR LOSS OF MY DAUGHTER" Thisis the big height till date (16th Jan ) they didn't send me any copyof document, No health wise card till date today..."May bethey are just waiting for another accident to happen ... andwaiting for rest of the members of family to vanish from the planet" Can't say about their intentions .......Because it is a BUSINESS forthem ..........and everything is fair in Busies.
At that moment I was so very muchpissed off by the reliance ......I spoke to myself " Mr..Ambani you don't know what wrong is going under your nose, itis ruining the name earned by Late Dhirubhai Ambani". Ifa person after taking a policy can not use it at the time when heneeds then what is the purpose of taking any kind of policy.
I am just trying to understand...what exactly happened ....Even after paying the premium amount ....can't they process a card and document with in 15 days. How longa courier takes to reach from point A to point B in India. Offcourse not infinitely( Because till date I have not received any post).
I asked myself ....Wasit a Charity ? Was it a donation .......that I did on 30th November ..what was that ....I can't understand. Even when I renewed thepolicy on 30th November 2010, I should have received a written copyof the policy with in 15 days. Simple case of fraud. Reliance killedmy daughter.

a) I Personally called themon 31st December to get the information about her. With old policynumber I got nothing, I was referred from one call center number to other.Then I remember that I had received the SIMS of transaction done on 30thNovember. I pulled out the SMS from my old hand set, and here I gotthe SMS information. I am typing the exact text that I received "Thankyou for choosing Reliance General Insurance. Your Health Wise Policy Numberis 1304702825030378. You can view your policy and take a print by loggingon http://imd.reliancegeneral.co.in after48 Hours". I got the similar kind of SMS for Policy number 2914510303781& 2914510303782. When I tried to download the policy online Igot an error ( Invalid policy number). This was the shatteringmoment for me ......A big Jolt .....
b) I sensed the conspiracyby Reliance, and kept on calling the customer care keeping my head cooland not declaring that my daughter is expired. I was afraid of tellingit, because till that date I had nothing in my hand. Just three SMS, whichsays check online ...download online .....and when I tried to download....I got a slap on my face. I realized that ...something has gone wrongwith RGI(Reliance general insurance) Either they have not renewed my policyor ...they have goofed up something. I don't wanted my self to me victimof their cunningness ....I kept on asking for Hard copy.
Since 30th December 2010 weare asking RGI to send a hard copy as well as soft copy of the Policy.Can you realize how long it takes to send a soft copy of the policy onemail. Does it takes more than 5 days. Whenever I use to call customercare ....they will tell me ..."Sir you will get the soft copy of thepolicy with in an hour I am forwarding it". It took 5 days to forwarda soft copy of policy. I got it on 3rd of January 2011. And till date Ihave only received the soft copy. On enquiring about hard copy, we gota reply that "it has been dispatched on 31st December,then we got a reply it has been dispatched on 3rd of January, and the latestis It has been dispatched on 5th of January" every time weuse to call, this dispatching date changes.
For the first time on 3rdof January we got a complaint number, customer care acceptedour complaint. Its Complaint number is 1661171. I don'tknow under what section they have placed it. Till date there is no Hardcopy from Reliance, and no Card for Health wise policy.
Here are call details : -
I) First talk on Date31 December to customer care service, tried to download the policyfrom net. It got failed.
2) Tried the same thingon 1st of January as well it failed, tried to call local relianceperson.
3) On 3rd January called customercare Reference number 1654845,Complaint number 1654847. Informedby customer care that courier has been dispatched on 31st By ExpressIT and Docked numberis 2915405091.
4) On 5th January calledcustomer care at Time5.25 PM Person Mr.Gaurav Call reference number 1661086 .
5) On 5th of January 11 calledcustomer care at Tme5.41 Person AnitaCall reference number 1661171.
6) On 10th Of January Time 1.50 , 1.56 Person Savitha& Rajesh Call refrence number 1668988. ( Call was droped by customercare executive blatanly).
7) On 13th January Time3.30 Minutes Refrence number 1654845 (Oldone) ( Clearlydenied to provide us the call reference number ). They also provided usa new Docked number for the courier dispatchedAWB2975619592
Now when their call centerpeople knew something has went wrong with this policy, these people stoppedresponding to us. They even started dropping calls( We have all the callhistory and can bring that too in court), whenever we use to ask for referencenumber of the call, they will disconnect the call.
After getting pissed off fromthe calls, I started sending email ( To the email id which I got from reliancewith the soft copy of the policy services.rgicl@relianceada.com,)asking about the contradiction in the policy statement.
Here is the contradiction :- The table below says CSI is 10 Lakhs in case of Accidental death &the warranties/Conditions contradicts it. ?

To get the clear picture aboutthe policy and to go ahead with Accidental death claim ... Ikept on sending email starting from January 6 till January 12 on services.rgicl@relianceada.com, I have attached the whole email sequence in zip file. Till date we havenot received any reply ....a single reply to this email.
This is a super height ofinsensitivity ...in human behavior. They are just treating us as a streetdogs .....after taking money ....no use ....just wanted to throw usin Trash. In my email below even after I warned to take a legal action,I got zero reply from them. [Please see the attached email below ...a big silence from Reliance ]
This email is for to claimunder accidental death ..and I am requesting IRDA and otherconcern people to take this case in a serious matter. I will fight tillmy last breath .....I suffered ... I don't want any other father to loosehis daughter like this ......I don't want others to suffer ......
I am sure ....Prakrati mustbe laughing at us .....She knew about the dirtiness on the Planet ....Henceshe decided to leave the planet............
